This form is used for a reservation request for a current Odd Fellow Member. You can only request a reservation for yourself and no one else.

This form is for an Odd Fellow Member who is requesting a reservation for a guest. All guests must be accompanied by the member registering. A member can only allow a potential member/guest, 2 (two) visits at the park. After that they must join Odd Fellows to use the facilities.

This form is for members who have a current reservation date and would like to cancel. If you are canceling your reservation with less than 72 hours notice, please call 541-997-7546 and speak with park staff.
